All N One Home Health Agency is looking for an experienced full time Case Manager/Home Health RN and per diem Home Health Nurse. This is a salaried position, 60K to 62K per year depending on experience. Per diem is DOE. Must have the following:
Current state or compact license to practice as a Registered Nurse
Minimum of 2 years experience, one year as an RN and one year experience in Home Health/Hospice
Current CPR certification
Must be knowledgeable in the use of the OASIS form
Must be able to document accurately
Must understand how to do a Start of Care
Must understand how to do follow-up visits
Must be able to do Discharge planning
Must have reliable transportation, current driver's license and automobile liability insurance
Must be able to travel
