We are a Real Estate Company that manages several Apartment Communities throughout the state of Arizona. We are currently looking to onsite Case Manager/Supportive Services of a Low-Income Senior Community. This position is a full-time, 40-hour a week, Mon-Fri 8am-5pm position. Benefits eligible after 90 days.
The onsite Case Manager is responsible for, but not limited to, providing supportive services to 30 of the Residents- coordination between health, social services, and charitable entities. Keeping in communication via regular meetings and recording data into software systems tracking resident information.
